6/27/2023 EnchantaVaughn Approved Upon arrival to the home setting, QMHP-A inquired about Kailaâs mental health. QMHP-A inquired about any mental health emergencies during session. QMHP-A asked Kaila if she had felt any mental health triggers. Kaila was well groomed and prepared for the session. QMHP-A informed Kaila the topic of this session ways to continue to manage her depression. Kaila did not present any symptoms of S/I or H/I at the time of the session. QMHP-A assisted Kaila with identifying and implementing useful conflict resolution skills that can assist with avoiding conflicts by using calming/ relaxation techniques such as deep breathing, walking away, active listening, positive eye contact and appropriate responses. QMHP-A used role-playing to assist Kaila with processing effective means of resolving conflict. QMHP-A shared personal experiences with Kaila to help her identify connecting with family members, developing new peer relationships and spending time in social settings. QMHP-A assisted Kaila with identifying and implementing change that is positive, manageable and reasonable for all involved though behavioral management. Kaila shared that she continues to need help with being able to talk to people and responding as well. Kaila acknowledged there are areas in her life that challenge her. Kaila was encouraged to use calming and relaxation skills taught. Kaila was supported for her agreeing to use those calming techniques when needed for resolving conflict. QMHP-A encouraged Kaila to use self-monitoring, continue developing positive affirmations and using positive self-talk. Kaila appeared receptive to learning and developing strategies such as self-control, self-talk that is positive, problem solving, relaxation techniques and thought- stopping to help manage her behaviors.
